Laparoscopic Management of Perforated Appendicitis

With the advance of laparoscopic instruments and accumulated experience in using Laparoscopic appendectomy for uncomplicated appendicitis, Laparoscopic appendectomy has been more frequently used for perforated appendicitis.
Perforated appendicitis is a severe complication of acute appendicitis that occurs when the inflamed appendix ruptures, leading to contamination of the peritoneal cavity. Traditionally, open appendectomy was the standard approach for managing perforated appendicitis, but with the advancement of minimally invasive surgery, laparoscopic appendectomy has emerged as a safe and effective alternative.
Clinical Presentation
Patients with perforated appendicitis often present with:
Severe abdominal pain, often generalized after initial right lower quadrant pain
Fever and signs of systemic infection

Laparoscopic Approach
Laparoscopic management of perforated appendicitis has several advantages, including reduced postoperative pain, shorter hospital stay, faster recovery, and better visualization of the peritoneal cavity for cleaning contaminated areas. The procedure generally involves:
Patient Positioning: Supine position with slight Trendelenburg tilt to facilitate exposure.
Port Placement: Typically, three ports—one umbilical port for the camera, and two working ports in the lower abdomen.
Exploration: Careful inspection of the peritoneal cavity to locate the appendix and identify any abscess or peritoneal contamination.
Appendix Removal:
The mesoappendix is divided using bipolar cautery or energy devices.
The base of the appendix is ligated, commonly with endoloop, intracorporeal sutures, or staplers.
The appendix is removed using an endoscopic bag to prevent spillage.
Peritoneal Toilet: Thorough irrigation of the peritoneal cavity with warm saline to remove pus and debris.
Drain Placement: A drain may be placed in cases of significant contamination or abscess formation.
Postoperative Care
Broad-spectrum antibiotics are administered to cover common gastrointestinal flora.
Early mobilization and gradual resumption of diet.
