PCOS responds favourably to laparoscopic ovarian drilling surgery
Recent sudy has shown that women with polycystic ovarian syndrome respond favourably to laparoscopic ovarian drilling surgery. The success rate of laparoscopic ovarian drilling in women with infertility duration of less than three years, treated with ovarian drilling with diathermy, in whom the pre-opre-operative luteinising hormone levelslevel was more than 10 IU/L reached 79%. Laparoscopic ovarian drilling is a minimal access surgical treatment that can trigger ovulation in affected reproductive age group women with polycystic ovary syndrome (PCOS). Electrocautery or a laser is used to destroy parts of the ovaries.
Women with PCOS who conceived following the surgery had a shorter duration of infertility, if they were treated with diathermy rather than laser, and these woman had higher pre-operative luteinising hormone levels, were younger and were more likely to have ultrasonographic proved evidence of long duration bilateral polycystic ovarian disease. Many study which has been done at differenet centre and multiple regression analysis showed that the duration of infertility, modality used in treatment either laser or diathermy and the pre-operative levels were the main determinants of the outcome. Studies of women with PCOS have shown that ovarian drilling results in an 80% ovulation rate and a 50% pregnancy rate.

Ovarian drilling is sometimes used for women with PCOS who are still not ovulating after trying weight loss and fertility medicine. Destroying part of the ovaries has been reported to restore regular ovulation cycles. After ovarian drilling live birth rates, which are the true measure of treatment success, are not available and are probably less than 50%. There is no any randomized controlled trials have been done used to study this treatment for starting ovulation if it is completely absent.
