Laparoscopic Management of Endometrioma
Laparoscopy is easily the most common procedure employed to remove and diagnose mild to moderate endometriosis. As opposed to employing a large abdominal incision, the surgeon inserts a lighted viewing instrument called a laparoscope using a small incision. If the surgeon needs better access, he or she makes one or two more small incisions for inserting other surgical instruments.
There are several methods for surgically treating an endometrioma, including draining it, removing component of it, or removing it entirely (cystectomy). Any of these treatments brings pain alleviation for almost all women yet not all. Cystectomy is most probably to relieve pain for an extended time, prevent an endometrioma from growing back, and stop the need for another surgery
The laparoscopic surgical procedures are method preferred by endometrioma. A combined technique (stripping and ablation) has proved never to be deleterious on the ovary. Understanding of endometriosis being a disease with substantial morbidity is truly essential. Laparoscopic treatment of endometriosis is useful for reducing pain and improving fertility.
Preoperative hormonal suppression with gonadotropin-receptor hormone analogue can be useful when you are decreasing endometriosis disease scores. Postoperative hormonal suppression with either a gonadotropin-receptor hormone analogue or progestin (including the levonorgestrel intrauterine system) can be helpful in reducing pain and increasing time to recurrence of symptoms. Excisional cystectomy may be the preferred approach to treat endometrial cysts both for fertility and pain and could be aided using mesna and initial circular excision.
Laparoscopy can be used to examine the pelvic organs and also to remove implants and scar tissue. This treatment is generally used for treating and checking:
- Severe scar and endometriosis tissue that may be considered to be upsetting body organs, such as the bowel or bladder.
- Endometriosis pain containing continued or returned after hormone therapy.
- Severe endometriosis pain (some women and their doctors opt to skip medicine treatment).
- An endometriosis cyst upon an ovary (endometrioma).
Endometriosis as being a possible reason behind infertility. The surgeon usually removes any visible implants and scar tissue. This might improvefertility.
