Total laparoscopic hysterectomy (TLH) is the complete hysterectomy including transection of the uterine vessels and opening/closure of the vaginal vault performed laparoscopically. This procedure can be performed as an alternative to total abdominal hysterectomy in many cases. The procedure was carried out using the ICG illumination. An illuminated manipulator was used due to difficulties in identifying vaginal vaults and small uterine mobility, most likely after inflammation. The left side of the uterine artery was ligated by Mishra's knot.

Mishra’s knot has been proven to be one of the most secure, safe extracorporeal knots in laparoscopy. It can be used for all continuous tubular structures of up to 22 mm diameter.
The technique is easy to perform and can be done in minimal possible time. “This technique of extracorporeal knotting is simple, easy, and reproducible with a good knot and loop security and can be used with any suture material of any size.”

Hysterectomy or removal of uterus is one of the commonly performed operations in Gynaecology. It was most commonly done by abdominal route since ages. But open hysterectomy with big incision and later on ugly scars in the abdomen has its disadvantages of late recovery, more pain, more infection, more loss of official work days and hernia and is obviously of no cosmetic benefit.
