Total laparoscopic hysterectomy (TLH) is a minimally invasive surgical procedure used to remove the uterus and cervix through small incisions in the abdomen. Here is an overview of the procedure:

Port placement: Several small incisions will be made in the abdomen to allow access for the laparoscope and other instruments. Typically, four to five ports will be placed.

Dissection of the uterus and cervix: The laparoscope will be inserted through one of the incisions to allow the surgeon to view the uterus and cervix. The blood vessels and tissues attaching the uterus and cervix to surrounding structures will be dissected and sealed using specialized instruments, such as a bipolar or ultrasonic energy device.

Removal of the uterus and cervix: Once the uterus and cervix have been completely dissected, they will be removed through one of the incisions. The vagina will be sutured closed where the cervix was removed.

Closure of incisions: The incisions will be closed with sutures or surgical glue.

Recovery: The patient will be monitored in the recovery room until they wake up from anesthesia, and then will be transferred to a regular hospital room. They may experience mild pain or discomfort, which can be managed with medication.

Total laparoscopic hysterectomy is generally associated with less postoperative pain, shorter hospital stays, and quicker recovery times compared to traditional open surgery. It also results in less scarring and blood loss. However, as with any surgical procedure, there are potential risks and complications, such as bleeding, infection, and injury to surrounding organs. - Vaginal Hysterectomy: The uterus is removed through an incision inside the vagina, leaving no external scars.
- Laparoscopic or Robotic Hysterectomy: Small incisions are made in the abdomen to insert specialized instruments and a camera for minimally invasive surgery.
 
4. Removal of the Uterus
The surgeon carefully detaches the uterus from surrounding structures, including the ovaries, fallopian tubes, and ligaments. Depending on the case, other reproductive organs may also be removed. For instance, in a total hysterectomy, the cervix is also excised, while a radical hysterectomy may involve removing nearby lymph nodes or tissues.
 
5. Securing Blood Vessels
During the procedure, blood vessels supplying the uterus are carefully clamped and sealed to prevent excessive bleeding. Advanced tools like surgical staples or electrocautery devices are often used for precision.
 
6. Closure and Recovery
Once the uterus is removed, the surgeon closes the incisions. In abdominal surgery, sutures or staples are used for the external incision, while internal stitches are placed for vaginal or laparoscopic methods. A sterile dressing is applied, and the patient is moved to the recovery area for monitoring.
 
7. Postoperative Care
