Articulating instruments are very useful and help to perform complex tasks in da Vinci Robotic Surgery. Can we perform laparoscopic Surgery with an articulated Instrument? Can you suggest any instrument available which I can use? |

Hi Dr Yadav There are many laparoscopic articulating instruments are available which you can get on the market to increase surgical efficiency. Handheld articulating laparoscopic instruments are expensive and still not manufactured by every instrument company. One brand is Kymerax, a new robotic-driven, handheld articulating laparoscopic device, that provides improved triangulation while preserving precise motions, which has been introduced to the European market and now used extensively. The ArtiSential line of articulating laparoscopic instruments is also available which is designed to provide fully articulating, wristed motion during laparoscopic surgery. The instruments are constructed with two pinching triggers on the handle controlling opening and closing, and the wrist controlling the articulating instrument head. Movement of the surgeon’s hand, wrist, and fingers are perfectly matched and translated into corresponding movements of the end-effector within the working field. The double-joint structure of the Artisental end-effector allows the instrument to move with 360o of freedom and provides control not available with conventional laparoscopic instruments. The current instrument lineup includes bipolar fenestrated forceps, a short needle holder, a bipolar Maryland dissector, and a clip applier. There are several devices with articulation such as Cambridge Endo and Novare RealHand. However, there are no studies evaluating these devices. The Endoeye® Flex 3D system (Olympus, PA, USA) was used for the new multi-DOF articulating instrument in order to balance the laparoscopic view just like the robotic system had a built-in 3D imaging system.
