I want to know the advantage of open surgical sacrocolpopexy for vault prolapse is better or laparoscopy is better? |

Dear Dr.G.SSALI, Laparoscopic sacrocolpopexy has more advantages in terms of reduced morbidity, shorter hospital stay, and convalescence.By laparoscopically there is easier access and placement of the mesh down the posterior vaginal wall compared with the open procedure. During laparoscopic sacrocolpopexy vaginal vault was elevated with a vaginal probe than the peritoneum was incised by scissors to expose the rectovaginal and vesicovaginal fascia, extending to the sacral promontory. One piece of type 1 polypropylene mesh was attached anteriorly and another as far down the posterior vaginal wall as possible. Than mesh was attached to the sacral promontory using staples.After that mesh was peritonealised at several points. |

Dear Dr. G. SSALI, Genital prolapse is one of the very common problem in women. Women who had 4 or more vaginal deliveries have 12 times more risk of genital prolapse. In theory laparoscopic approach to the repair of the vault prolapse follow the same principle as in the open technique, Vault prolapse repair is based on use of native tissues or synthetic materials. In my opinion laparoscopic sacrocolpopexy should be considered as gold standard for all types of genitourinary prolapse in female. With regard Dr R K Mishra |
