Dear Dr Sakhuja The harmonic scalpel is trade name for Ethicon ultrasonic generator which is an ultrasonically activated surgical instrument system used extensively in laparoscopic surgery for tissue dissection. It has shown fast expanding surgical applications, there are no much reports about associated complications but if the vibrating jaw will directly touch the bowel or vessel it can puncture it. Acoustic energy coupling produced by vibrating jaw of harmonic scalpel and overheating of its active blade extender sheath occur with bending of the instrument. This can happen with steering of the blade extender during laparoscopic surgery and may increase the exposure risk of adjacent tissues to injury with temperature sometime rises to 180 degree centigrade. This happen due to friction of metal to metal. |

Harmonic scalpel also has the problem that it does not last for more than 5 surgery and it is too expensive. |
