Thank you for the information. Unfortunately the youtube related links are blocked here(Iran). Yet, I will try to get the videos by some tricks. so far I was unsuccessful.

Hi,

Thank you for the URL - As a matter o fact the video was made in Brigham Women Hospital. My advisor of the presentation is Dr. Pozner the medical director of simulation center of Harvard Medical School too. I also found some pages on this subject in Dr. Nezhat's book. He will be in the congress too.

1-I have a question; with either unidirectional (V-Loc) or bidirectional barbed suture, the end (tail) of the suture is just cut? In one video it was locked and then cut. Is it enough?

2-The barbed suture can only be used in cuff closure of hysterectomy procedure, It can not be used for uterine artery pedicles , can it?

3-Is barbed suture more expensive than smooth suture? is it shorter by length?

4- can it be used in for example episiotomy?

5- Is it popular in your center, or does it need more approval for safety ?(in Iran for example- I mean at a local level)

Sorry for late response and wishing you first very happy new year. Personally I am against the barbed suture. In my opinion it is more industry driven than its real use. You can see few adverse effect published and currently it is in scrutinize process of FDA. Few obstruction and fistulization has been reported. The reason to account for the small bowel obstruction was the presence of free barbed suture in the peritoneal cavity as it invited adhesions formation and subsequently the bowel obstruction and fistulization. So the free end should be cut. If you are tying a knot there is no need of barbed suture.

Thank you for the reply. It helped a lot.
As you said, my impression from your answer is,
1- the end should be cut , as it can cause adhesions
2- its safety is in question so it is risky to use barbed suture on vascular pedicles
3- it is not popular, it is more expensive
4- it needs to be reevaluated and approved by local health administrators
