Post cholecystectomy pancreatitis

2 patients operated in the last month (Lap cholecystectomy) presented with sudden onset pain in the right hypochondrium radiating to back.On evaluation found to have mild raise in the lipase level,(amylase is normal,LFT normal,CBP is normal).Pain is relieved by antispasmodics.What could be the cause of this mild pancreatitis 1 month after surgery?

Dear Dr Rajneesh

It is possible that after laparoscopic cholecystectomy microliths in the CBD bile may have passed and crossed the sphincter of Oddi causing temporary obstruction with precipitation of an attack of acute pancreatitis.

In those cases MRCP should be performed. Typical fluid collections in peri pancreatic area with fat stranding are diagnostic of pancreatitis. Prompt and aggressive supportive care can lead to early
resolution of pancreatic inflammation.
