Regarding Trocar size used fot Extra-corporeal Knot in Laparoscopic Surgery?

Sir, for extracorporeal knitting, Mishra's knot. or any technique...first accessory port size for suture passer is 10 mm or 5 mm..? Reducer to be used...Please also tell the first accessory port size in intracorporeal knitting......in my hospital nobody is using extracorporeal knot & Introduces needle directly..after removing port......almost all surgeon I have seen so much confused....please guide...... Thanks.

Extracorporeal knot need special training and it is very useful for tissue under tension and wherever there is difficult ergonomics. The intracorporeal knot is more versatile and every surgeon they know it as it is always used in open surgery. Of course for extracorporeal knot most of the time you need 10mm port whereas for intracorporeal you can directly push the needle with the suture through the wound. The extracorporeal knot is also ideal for all the tubular structure like appendix and any artery or vein because it makes a good dumble effect.
