My question is that for diagnostic laparoscopy can I use nitrous oxide? What are the advantages and disadvantages of using Nitrous Oxide in laparoscopic surgery? |

Dear Dr.John Makumbe, Use of Nitrous Oxide during laparoscopic surgery has been a matter of debate between anesthesiologists and surgeons worldwide, Nitrous oxide causes bowel distension during laparoscopic surgery because nitrous oxide can diffuse in the bowel lumen. When bowel distension occurs in the course of an operation it may increase surgical difficulty to a considerable degree. I think some surgeons use it because it is inexpensive, analgesic and short-acting. It is most widely used mixed with oxygen to reduce the concentration of other potent volatile anesthetics to minimize cardio-respiratory depression. |

Dear Dr John Makumbe Different gases have been proposed worldwide by laparoscopic surgeons for creating pneumoperitoneum with its own advantages and limitations. Carbon dioxide has been proposed and considered to be one of the safe gases for pneumoperitoneum during all type of advanced laparoscopic surgery. Nitrous oxide can be used under local anesthesia for Diagnostic laparoscopy but for operative laparoscopy it is not a good choice because it is combustible agent but for combustion to occur in a nitrous oxide environment, hydrogen or methane must occupy 5.5% of the gas volume. Worldwide current minimal access surgical practice is to use 100% carbon dioxide for pneumo-peritoneum, because it is not combustible and thus will not create an explosion even if the electrosurgical dissection generate a spark. |
