This video demonstrates Total Laparoscopic Hysterectomy by three 5 mm ports by Dr. R K Mishra at World Laparoscopy Hospital. It is surgery we have also used stryker Ukit for added safety. Total laparoscopic hysterectomy is a safe and effective procedure for women needing a hysterectomy. We enjoy a high operative volume and perform approximately 150 total laparoscopic hysterectomy cases annually with a conversion rate of 1:500. The steps described herein are not meant to be an absolute truth, but rather a true and tested method that has served us well to safely accomplish this procedure.

Total Laparoscopic Hysterectomy by Three 5 mm Ports: A Minimally Invasive Advancement

Total Laparoscopic Hysterectomy (TLH) has revolutionized gynecological surgery, offering patients a minimally invasive alternative to traditional open procedures. Among the various laparoscopic techniques, TLH performed using only three 5 mm ports represents a significant advancement, providing improved cosmetic outcomes, reduced post-operative pain, and faster recovery.

Understanding the Three-Port 5 mm TLH Technique

This approach utilizes three strategically placed 5 mm ports, eliminating the need for larger incisions typically required in conventional laparoscopic hysterectomy. The ports are usually positioned as follows:

  1. Umbilical Port (5 mm): Acts as the primary access for the laparoscope, providing high-definition visualization of the pelvic anatomy.
  2. Two Lateral Ports (5 mm each): Used for precision instrument manipulation, ensuring effective dissection, coagulation, and suturing.

Unlike traditional techniques that may require a 10 mm port, this approach minimizes tissue trauma and enhances cosmetic results.

Advantages of the Three 5 mm Port TLH

Minimal Scarring: Smaller incisions lead to less visible scarring, making this an appealing option for cosmetically conscious patients.
Reduced Pain: Smaller incisions contribute to less post-operative pain, leading to reduced analgesic use.
Faster Recovery: Patients experience quicker recovery times and an earlier return to daily activities.
Lower Risk of Complications: The reduced port size decreases the likelihood of port-site hernias and infections.

Indications and Patient Suitability

This technique is particularly beneficial for patients with:

  • Benign uterine conditions such as fibroids, adenomyosis, or abnormal uterine bleeding.
  • Uteri of moderate size, allowing for safe laparoscopic manipulation.
  • No extensive pelvic adhesions, ensuring ease of dissection with minimal instrumentation.

Conclusion

Total Laparoscopic Hysterectomy with three 5 mm ports exemplifies the evolution of minimally invasive gynecologic surgery. By reducing incision size while maintaining surgical efficacy, this technique prioritizes patient comfort, recovery, and overall surgical outcomes. As technology and surgical expertise continue to advance, this approach will likely become a preferred choice for both surgeons and patients seeking optimal results with minimal intervention.
