This video demonstrates laparoscopic inguinal hernia repair by Dr. R K Mishra at World Laparoscopy Hospital. Laparoscopic inguinal hernia repair has got comparable results in comparison to open hernia repair. Many studies have shown that Laparoscopic inguinal hernia repair gives better results in terms of recurrence as compared with open hernia surgery but with the added advantage of fewer chances of post-operative, pain, wound infection, and early return to activity. Laparoscopic inguinal hernia repair was started using the transabdominal preperitoneal (TAPP) approach. Another technique of LIHR that has evolved is totally extraperitoneal (TEP) repair. An inguinal hernia is a bulge that occurs in your groin region, the area between the lower part of your abdomen and your thigh. Inguinal hernias occur because of a weakening of the muscles in the lower abdomen. Three layers protect the intestines inside the lower abdomen. The first is a thin membrane called the peritoneum. The second is made up of the abdominal muscles, and the third is your skin. An inguinal hernia forms when your intestines and the peritoneum push through the muscles and appear as a bulge under your skin. Inguinal hernias are dangerous because they tend to keep getting larger and your intestine can get trapped inside the bulge and lose its blood supply. This is called a strangulated inguinal hernia, and surgery may be needed to correct the problem.

Inguinal Hernia Surgery (TAPP for Indirect Inguinal Hernia)

Transabdominal Preperitoneal (TAPP) repair is a minimally invasive laparoscopic technique used to treat indirect inguinal hernias. This procedure involves making small incisions in the abdomen, allowing surgeons to access the hernia from inside the peritoneal cavity. A mesh is placed over the defect to reinforce the weakened area, reducing the risk of recurrence. TAPP offers faster recovery, less post-operative pain, and a quicker return to daily activities compared to open surgery. With its high success rate and minimal complications, it is a preferred choice for patients seeking an effective and efficient hernia repair.

An inguinal hernia occurs when a part of the intestine or fatty tissue protrudes through a weak spot in the abdominal wall, near the groin. Among the different types, an indirect inguinal hernia is the most common, typically present from birth due to an incomplete closure of the inguinal canal. Transabdominal Preperitoneal (TAPP) repair is a widely used laparoscopic technique for treating indirect inguinal hernias, offering excellent outcomes with minimal recovery time.

Understanding TAPP Repair
The TAPP (Transabdominal Preperitoneal) approach is a minimally invasive laparoscopic procedure that allows surgeons to repair the hernia from inside the abdomen. This method provides a clear view of the hernia and surrounding structures, making it particularly useful for complex or bilateral hernias.

Step-by-Step Procedure
1. Anesthesia & Preparation: The patient is placed under general anesthesia, and small incisions are made in the abdominal wall.
2. Laparoscopic Access: A camera and specialized instruments are inserted to visualize the abdominal cavity.
3. Peritoneal Incision: A small incision is made in the peritoneum (the inner abdominal lining) to access the hernia defect.
4. Hernia Reduction: The herniated tissue is gently pushed back into the abdominal cavity.
5. Mesh Placement: A synthetic mesh is placed over the defect to reinforce the weakened area and prevent recurrence.
6. Peritoneal Closure: The peritoneal flap is closed using sutures or surgical glue.
7. Incision Closure: The small skin incisions are closed, typically with dissolvable sutures.

Advantages of TAPP Repair
✅ Minimally Invasive: Smaller incisions mean less post-operative pain and faster healing.
✅ Lower Recurrence Rates: The use of mesh provides a durable repair.
✅ Bilateral Hernia Repair: Both sides can be treated in the same procedure if necessary.
✅ Faster Recovery: Most patients resume normal activities within a few days.

Recovery & Post-Operative Care
- Hospital Stay: Usually performed as an outpatient procedure or a short hospital stay.
- Pain Management: Mild discomfort can be controlled with over-the-counter pain relievers.
- Physical Activity: Light activities can resume within a few days, but strenuous activities should be avoided for 4–6 weeks.
- Follow-Up: Regular check-ups ensure proper healing and prevent complications.

Potential Risks & Complications
While TAPP repair is generally safe, potential risks include:
⚠️ Infection (though rare due to minimally invasive techniques)
⚠️ Bleeding or bruising around the incision sites
⚠️ Mesh-related complications, though uncommon
⚠️ Temporary groin discomfort or numbness

Conclusion
TAPP surgery is a highly effective, minimally invasive method for treating indirect inguinal hernias. With faster recovery times, reduced pain, and lower recurrence rates, it is an excellent choice for patients seeking a modern surgical solution. If you're considering hernia repair, consulting with a specialized surgeon can help determine the best approach tailored to your condition.
