अप्रत्यक्ष वंक्षण हर्निया की IPOM तकनीक
विश्व लेप्रोस्कोपी अस्पताल में, डॉ। आर के मिश्रा ने 36 रोगियों में इंट्रापेरिटोनियल ओनेल मेष मरम्मत (आईपीओएम) का उपयोग करके लेप्रोस्कोपिक हर्नियोप्लास्टी पर अपने कर्मियों के अनुभव को विकसित किया है। इस तकनीक में मिश्रा की गाँठ का इस्तेमाल मेष को ठीक करने के लिए किया जाता है। विपोरो II जाल की सुटिंग तकनीक की थ्रिस स्टाइल में, सभी रोगी प्रत्यक्ष या अप्रत्यक्ष वंक्षण हर्निया से पीड़ित थे और उनमें से कुछ ही शामिल थे जिनके पास आवर्तक वंक्षण हर्निया था, जिनमें से 7 आवर्ती थे और 20 में एकतरफा हर्निया और 9 थे द्विपक्षीय वंक्षण हर्निया। हर्निया की मरम्मत का उपयोग विप्रो II मेश का उपयोग करके किया गया था और कृत्रिम ऊतकों को टाइटेनियम सर्पिल टैक (प्रोटैक, ऑटोसेवन, टायको हेल्थकेयर) के साथ तय किया गया था और कुछ को कॉडियन एब्जॉर्बेबल टैक्सेबल एब्सोर्बटैक ™ (एबीएसटीके 30 एक्स) फिक्सेशन डिवाइस के साथ तय किया गया था।
विश्व लेप्रोस्कोपी अस्पताल, गुड़गांव, भारत में किए गए हमारे अध्ययन में, कोई इंट्राऑपरेटिव जटिलताएं नहीं हुईं और कोई रूपांतरण आवश्यक नहीं था। इस अध्ययन में सेरोमा के गठन से पहले एक सप्ताह के लिए चुस्त समर्थन दिया गया था। मतलब अस्पताल में 24 घंटे रहा। दो सप्ताह के भीतर काम पर लौटने के साथ सामान्य गतिविधि की बहाली 7 दिनों की थी।
6 महीने के औसतन, 1 पुनरावृत्ति दर्ज की गई। इस अध्ययन के परिणामों के साथ-साथ साहित्य में प्रस्तुत श्रृंखला के मेटा-विश्लेषण से संकेत मिलता है कि आईपीओएम वंक्षण हर्निया सर्जरी आवर्तक और द्विपक्षीय हर्निया के उपचार में एक व्यवहार्य, सुरक्षित और प्रभावी प्रक्रिया हो सकती है जब हर्निया की मरम्मत होती है अन्य लेप्रोस्कोपिक प्रक्रियाओं के दौरान प्रदर्शन किया।
IPOM को अन्य पारंपरिक रूप से किए जाने वाले पारंपरिक TEP और TAPP लैप्रोस्कोपिक हर्नियोप्लास्टी की तुलना में तेजी से और आसानी से ठीक होने के लिए दिखाया गया है। ये डेटा इस तकनीक को विशेष रूप से आदिम हर्निया के मामलों में उपयोग करने का सुझाव दे सकते हैं जैसे कि बिना किसी कोमोरिटी के बहुत सक्रिय युवा पुरुष। इस प्रकार की हर्निया की मरम्मत करने का हमारा अनुभव बहुत उत्साहजनक है। हालाँकि सीमित श्रृंखला और लघु अनुवर्ती एक बड़े यादृच्छिक भावी दीर्घकालिक अध्ययन के लिए निश्चित रूप से पुनरावृत्ति की सच्ची घटना और इसलिए इस आकर्षक प्रक्रिया की प्रभावशीलता का पता लगाने के लिए कहते हैं।
