maricel parado	i spoke 3doctors, they said that i cant use rowachol....or any meds to minimize the stone,,they want surgery, i refuse afraid with surgery

Dr M.K. Gupta	Dear Parado It is true that you need to go for surgery. You should not be afraid because now with laparoscopic surgery you do not get much pain. Knowledge of the basic pathophysiologic mechanisms underlying gallstone formation is really a prerequisite for successful treatment. Over 85% of gallstones under western culture are cholesterol stones. Our knowledge of the pathophysiologic mechanisms leading to cholesterol gallstone formation has significantly improved during the last 35 years. Indeed, to date, only cholesterol gallstones are amenable to medical dissolution therapy. Cholesterol gallstones are comprised of cholesterol monohydrate crystals (70% to 100% by weight) held together by a glycoprotein-rich matrix. They form due to the precipitation and aggregation of cholesterol monohydrate crystals within the gallbladder. Normally, biliary cholesterol is kept soluble within bile salt-rich micellar and phospholipid-rich vesicular lipid aggregates. Gallbladder stones are extremely common in developed countries and many developing countries including India and their prevalence increases appreciably with age; it is also based on sex, bodyweight, and race. Fortunately, of all occasions, a gall stone doesn't cause severe symptoms such as pain and jaundice. They are best detected by ultrasound. Troublesome gall stones are generally treated by cholecystectomy (gall bladder surgery involving its removal combined with the stones), and many of these procedures are performed by laparoscopy. Medical terms employed for gall bladder stone disease are cholelithiasis, chronic cholecystitis, acute cholecystitis, choledocholithiasis etc. Gallbladder ? Why we have a gallbladder at all is something of a mystery. Many animals for example horses, pigeons, and rats manage very well with out them. The gallbladder functions being an optional store for that bile secreted through the liver. Bile acids are important in making the fats within the intestine soluble before their digestion and absorption. The gallbladder contracts to eject high concentrations of bile in to the intestine when a fatty meal is consumed however it does exactly the same thing when any other kind of meal is taken or even when water is drunk. The gallbladder is clearly not required to human digestion since patients who have had a cholecystectomy don't have any problems in this regard. In fact, sometimes it seems as if its main role would be to keep doctors busy!! Along with bile acids, the liver secretes cholesterol and bilirubin into the bile, and herein lies the major problem. Cholesterol is not always in stable solution, even just in perfectly healthy people, and can crystallize to form stones. Additionally, the soluble conjugated bilirubin may be converted to free bilirubin, which then precipitates. The bile in the gallbladder thus remains an unstable concentrated "soup of chemical problems." It is the presence of stones in the gall bladder that explains almost all of the ills inflicted with this organ. The entire process of stone formation is believed to become the effect of a triple defect: Patients with gall stones have much more unstable bile compared to rest of us They have more sluggish activity of the gallbladder, that allows crystals to grow into large stones They are also prone to nucleate crystals in the first place, thereby initiating the process. The main element of gall stones is cholesterol, that the liver clears from the body via the biliary tract and intestine, but bilirubin pigment on its own can build stones, and calcium salts also join in the process to a varying degree. Who gets Gallbladder disease ? The picture on left is ultrasound showing stones and on right the same gall bladder exposed to show the stones. The prevalence of gall stones within the population can be measured conveniently by ultrasonography, and the prevalence of gallbladder disease is taken because the amount of individuals with gall stones and those who have had a cholecystectomy. Gallstone disease is becoming more prevalent in the western world in the second half from the Twentieth century. This is partly due to the ageing population, but probably for some other reasons too, for example increasing obesity within the population (figure). Doctors who believe that gall stones really are a disease of older women are mistaken. Though no definite theory is there to explain this phenomenon, the most popular hypothesis is the changing dietary habits. Genetically, the Indian body for hundreds of years, can be used to some low calorie and fewer refined diet. The incidence appears to be higher amongst population where there is a dramatic alternation in lifestyle in one generation. People moving from rural to urban areas not even close to their origin also seem to be more susceptible. Although no gene continues to be identified, the disease does appear to run in families. Pregnancy increases the chance of gall stones, but the oral contraceptive pill does not. While there is evidence the high doses of oestrogens found in hormone replacement therapy could cause stones, we do not yet have definite proof this is the case. There is general agreement that obesity causes stones, but much more same with the rapid fat loss caused by a stringent diet or bariatric surgery. No specific food continues to be incriminated within the formation of stones. Nevertheless, vegetarians have fewer stones. It has additionally been proposed that moderate quantities of exercise, aspirin, and non-steroidal anti-inflammatory medicine is protective. There is an international league table of gallstone prevalence. The United States and Western Europe lead the table while African countries are at the underside. Gallstone disease tends to be a disease more affluent societies, but there are clearly racial differences. However, the prevalence of gallstone disease in India has grown from a low level due to westernization of lifestyle, suggesting that environmental factors are essential. waseef	Sorry for offtopic, I've been investigated that i've 8mm gallbladder stone, now doc hav advised medicine for 3 months, my question is medicine a remedy or i need to remove the gall bladder Dear Waseef, Generally Medicines does not work for Gall bladder stone. If you have symptoms due to these stones you should get Laparoscopic cholecystectomy done, with regards Nidhi

Creatinine - 1.0 CNS : WNL Dear Vasudha Ureteric stone of 4.5mm size will come out spontaneously. Kidney stones are common, affecting 0.2% of adults annually. Approximately 7% of men and 3% of women will develop a kidney stone in their lifetime. Men develop kidney stones 3 to 4 times more commonly than women, and this is thought to be due to females having higher levels of urinary citrate, which is a potent stone inhibitor. The size of the stone helps predict the likelihood of the stone passing spontaneously. As a general rule, stones less than 4 mm in size have an excellent chance of passing by themselves and so intervention is only recommended in the presence of severe pain or infection. Likelihood of spontaneous passage of a stone by stone size (within 1 month): 4 mm - 90% 5 mm - 50% 6 mm - 10% If a stone does not pass spontaneously, or is not likely to, then treatment is recommended. Occasionally medicines may be offered to improve the chances of your stone passing spontaneously.

zulfiqar ali	Dear Sir, My age is 57 years and working in a office as Secretary. I checked by ultrasound that glad bladder shows stones about 8-9 cm. But I have no problems in stomach. Kindly advise or I will be operated or Do not need to go for operation. Or to be used any medicines. Thanks Dear Muhammad Yes laparoscopic cholecystectomy is the surgery that is performed in this type of case. It is done by putting a telescope inside the abdomen and the gall bladder with the stone is taken out. with regards

kokila	hi , i have two calculi measuring 5mm and 7mm in size in gall bladder. im facing pain under my left rib cage off late. alongwith frequent urination. please suggest the medication required. Dear kokila You have a stone in your gall bladder. Medicine is not the treatment for this. You need to go for the surgery laparoscopic cholecystectomy is the surgery of choice.
