Dear Anupam Kumari Cost of surgery for chocolate cyst at our hospital would be approximately 30,000 rupees in our World Laparoscopy Hospital, Gurgaon. Ovarian Chocolate cysts are enlargements from the ovary that appear to be filled with chocalate like fluid. They can be an easy fluid filled bleb or contain complex internal structures. The term cysts are used to differentiate them from solid enlargements. Simple cysts have no internal structures and therefore are less worrisome compared to those with complex structures or solid components. A sonogram or ultrasound test can determine if a cysts are simple or complex. Ovarian cysts are often encountered. Every menstruating woman develops cysts on the ovaries each cycle. The menstrual period necessitates the coordinated functioning from the pituitary gland, ovary, uterus and the cervix. The anterior pituitary gland in the head produces the endocrine system, Follicle Stimulating Hormone (FSH) and Luteinizing Hormone (LH). These hormones are freed in to the bloodstream and therefore are the messengers that tell the ovary how to proceed. The ovary responds to FSH by maturing an egg. While carrying this out the ovary makes and releases the hormone estrogen. The fermentation egg is within a follicle cyst that grows to about half inch in diameter. Once the pituitary releases a surge of LH the follicle breaks and also the egg floats out and eventually enters the tube. The remnant of the follicle cysts are known as the corpus luteum and makes the hormone progesterone. The lining from the uterus is stimulated to proliferate and grow under the influence of estrogen. Progesterone converts the liner of the uterus to become a secretory lining that's prepared to accept a pregnancy. If a pregnancy doesn't happen that cycle the corpus luteum stops making progesterone. Once the progesterone level falls the support for the lining from the uterus is lost also it sloughs. This is the menstrual bleeding. Then the cycle starts once again: estrogen, ovulation, progesterone and also the period. Sometimes the ovary does not ovulate and the follicle cyst persists. It'll continue to enlarge and can become as big as a baseball. Eventually it will break and also the woman may not even be conscious that this has happened. The period may be delayed since there is no progesterone phase from the cycle to reply to. The corpus luteum can also become cystic. Chocolate cysts are gradually separated from surrounding omentum & intestines & tubes. Chocolate cyst is separated from pelvic wall and uterosacral ligament and chocolate fluid is drained gradually to prevent spillage. Cystectomy is done with no damage normal ovarian tissue. Endometriotic lesions are excised if pain may be the main complaint and fulgurated with bipolar desiccation if Infertility may be the main complaint. Suction & irrigation done with Ringer lactate and Haemostasis is checked. Edges of the ovarian capsule are grasped with grasper for 2-3 minutes so ovary is restructured & Tubo-ovarian relationship established. Please do not hesitate if you have any further quesry. I have left ovarian chocolate cyst.. If i remove my overy by surgery.. In future can i pregnant ???? Dear Mrs Singh You don't need to get your ovary removed. Laparoscopic ovarian cystectomy can be performed safely without removing and damaging the ovary. Just after removing the cyst your fertility will not be impaired rather your chance of getting pregnant will improve. With regard Sadhana |
