What information do I need to bring with me to the Hospital?
Patients should bring the following information when registering for surgery at World Laparoscopy Hospital:
- Insurance Card
- Medicare/Medicaid card
- Name and Social Security Number of insured
- Referral forms from your physician
- Referral/Order Guidelines
- Worker’s compensation information if required
- Driver’s license or any photo identity
- Credid Card, Cash, Co-pay or deductible
- Pre-admission form if not previously submitted
