Laparoscopic and Robotic Fundoplication

 Definition 

Gastroesophageal reflux disease (GERD) is currently defined as a condition that develops when the reflux of gastric contents into the esophagus leads to troublesome symptoms and/or complications. GERD is the most common disease encountered by the gastroenterologist with a 20% prevalence in the adult population 

DIAGNOSIS

ERD and Barrett’s esophagus are found at endoscopy in up to 30% of patients with the typical reflux syndrome. Accordingly, endoscopic assessment is recommended only in the presence of alarm symptoms (dysphagia, vomiting, anemia, involuntary weight loss) and the diagnosis of GERD is currently patient-centered, 

PPIs represent the mainstay of medical treatment in GERD. They provide the most rapid symptomatic relief and heal esophagitis in the highest percentage of patients, transforming the vast majority of acid refluxes into weakly acidic refluxes.

Purpose

Laparoscopic fundoplication is currently a standard therapy for gastroesophageal reflux disease (GERD) with a sliding hernia. When compared to individuals who do not respond to medical therapy, those who do have the best outcomes.

Seventy patients (23 males and 47 females) with a mean age of 64 years (22-92) and a big paraesophageal hiatal hernia were treated with a robotic technique. For hiatal closure reinforcement, biosynthetic tissue absorbable mesh was used.

Method :

It is the most commonly performed fundoplication worldwide. During the procedure, the surgeon wraps the top part of the stomach around the esophagus to prevent the backflow of acids. A robotic-assisted procedure consists of several tiny incisions through which instruments and a fiber-optic camera are operated.

AFter Care

Avoid any foods that cause stomach gas and distention. These foods include corn, dried beans, peas, lentils, onions, broccoli, cauliflower and any food from the cabbage family. Avoid carbonated drinks, alcohol, citrus and tomato products.
