Abdominal minimal access surgery have been greatly advanced recently with the progression of new HD robotic technology and methods. In future at some time, all minimal access general and gynecological surgeries could have been done utilizing an open procedure. Nowadays open surgery is seldom used in big hospitals because open surgeries cause great trauma for the patient and cause increased pain, scarring, and downtime. Recently a shift toward Non-invasive Surgery also called Minimal Access Surgery has been gaining momentum. Even within MIS, the incision size and quantity of port incisions have been decreasing with every new advancement, however, with every new technique of laparoscopy, operating surgeon will find new constraints added to dexterity and visibility.  MORE

Life Care Medical Devices of USA won European regulatory clearance for its new laparoscopic access device called as KeyHole Cup. This device will probably helps to prevent internal organ damage during initial entry into the abdominal cavity in preparation for minimal access laparoscopic procedures using veress needle. The cup has unique sucking ability, using negative pressure. It will lifts the abdominal wall up and will keep the insertion point of abdomen away from the internal organs below, allowing for safer needle delivery just into the cavity. During Laparoscopic surgery access inside the abdominal cavity safely is most critical part. In reality there is no entry technique or device is absolutely safe. Avoidance of entry injuries of the bowel and vessels depends on patient-specific anatomic orientation and control of entry axial force. Certain good quality entry devices can be facilitating in controlling axial force. Overall, this large aggregate of risky entry access injuries shows them to be more serious for novice minimal access surgeon and, along with other data, implies that they might be more common than reported in procedure-based studies.  MORE

There is always a fear among gynecologist while performing laparoscopic ovarian cystectomy about Pseudomyxoma peritonei which can be a condition brought on by producing abundant mucin by tumor cells, which fills the abdominal cavity at the time of rupture of mucinous ovarian cyst. If not treated, mucin will ultimately build up to the stage where it compresses vital structures: the colon, the liver, kidneys, stomach, spleen, pancreas, etc. The disease rarely spreads from the the lymphatic system or through the bloodstream. It is seen as a tumors that leave mucin gelatinous ascites and scattered cancer cells from the abdominal cavity. This disease is mostly a result of an appendiceal primary cancer or cancer in the appendix, mucinous tumors from the ovary are also implicated, although generally ovarian involvement is favored to be a metastasis from an appendiceal and other gastrointestinal source. Pseudomyxoma peritonei is a very rare type of cancer that usually begins in appendix as a small growth and it can start in other parts of the bowel, the ovary or bladder. This eventually spreads through the wall of appendix and spreads cancerous cells to the lining of the peritoneum. These cancerous cells produce mucus, which collects in the abdomen as a jelly like fluid called mucin.

Laparoscopic Surgery is not a surgery in reality rather it is considered a surgery over image. Success and disaster of laparoscopic surgeon depends on if his imaging system is good or not. The 3D imaging system if used in laparoscopic surgery significantly improves spatial orientation and depth perception during laparoscopy under different surgical conditions. Conventional two dimensional (2D) laparoscopy has been limited by the lack of depth perception and spatial orientation. This disadvantage may affect surgical performance, operative time, or morbidity. Depth perception, spatial resolution, and accuracy might be improved by three-dimensional (3D) vision. These clinical studies conducted in different hospitals in different clinical setting was designed to appraise the effect of 3D imaging on the performance of novice and experienced laparoscopic surgeons, gynecologists and urologist for executing standardized laparoscopic tasks in an laparoscopic surgery operating room setting.   MORE

In recent study it has been found that da Vinci robotic gastrectomy can match the long-term clinical and oncologic outcomes of conventional laparoscopic surgery in patients with early gastric cancer, a large single-center study has shown. At the American Society of Clinical Oncology 2013 Gastrointestinal Cancers Symposium, researchers from Severance Hospital, part of the Yonsei University Hospital System in Seoul, South Korea, presented a retrospective analysis of their large prospectively designed database for gastric cancer.

da Vinci Robotic surgery does not appear, however, to provide superior outcomes than laparoscopy, which many experts in the beginning argued it is a question that how it we take the decision and robotic surgery will be necessary to justify the added costs of robotic surgery.  MORE

The goal of the Safe Laparoscopic Surgery Saves Lives Challenge is to improve the safety of minimal access surgical care around the world by ensuring adherence to proven standards of care in all countries. The WHO Surgical Safety Checklist has improved compliance with standards and decreased complications from laparoscopic surgery in pilot hospitals where is was evaluated. The entire time patients in Laparoscopic surgery commit to low blood pressure level might be a stronger predictor of serious postoperative complications as opposed to depth of the hypotensive troughs they hit throughout the procedure. Depending upon the final results of such validation steps, scientist anticipate an evolutionary practice change if not every, but many patients outcomes could be improved by minimizing hypotensive exposures during Laparoscopic Surgery. The investigators used logistic regression to discover the relationship between cumulative time spent beneath the various thresholds and 30-day mortality.  MORE
