I have had an ultrasound done. My ultrasound revealed multiple uterine fibroids. Will I Need Surgery? If So, What Kind?

Not all cases of uterine fibroids require surgery. Many women with fibroids may not experience significant symptoms or may have symptoms that can be managed with conservative treatments. If your fibroids are causing symptoms such as heavy menstrual bleeding, pelvic pain, or pressure, your healthcare provider may consider different treatment options:

Watchful waiting: If the fibroids are small and not causing significant symptoms, your healthcare provider may recommend regular monitoring without immediate intervention.

Medications: Certain medications, such as hormonal birth control or GnRH agonists, may be prescribed to help manage symptoms by controlling menstrual bleeding or reducing the size of the fibroids.

Minimally invasive procedures: Depending on the size and location of the fibroids, your doctor may suggest minimally invasive procedures like uterine artery embolization (UAE), myomectomy, or focused ultrasound surgery (FUS) to target and shrink the fibroids without removing the entire uterus.

Hysterectomy: In some cases, particularly if the fibroids are large, cause severe symptoms, or if you have completed your family planning, a hysterectomy (removal of the uterus) may be recommended.

Fibroids are noncancerous growths in the uterus that can cause symptoms like heavy menstrual bleeding, pelvic pain, and frequent urination. The type of procedure recommended depends on factors such as the size and location of the fibroids, your age, and whether you want to preserve fertility.

Some common procedures for treating fibroids include:

Medications: Hormonal medications such as birth control pills or gonadotropin-releasing hormone agonists may help relieve symptoms by regulating your menstrual cycle and reducing bleeding and pain.

Minimally invasive procedures:

Uterine artery embolization (UAE): This procedure involves blocking the blood supply to the fibroids, causing them to shrink.

Myomectomy: This surgical procedure involves removing the fibroids while leaving the uterus intact, which is a good option for women who want to preserve their fertility.

Laparoscopic or robotic-assisted myomectomy: These minimally invasive procedures involve removing fibroids through small incisions in the abdomen, resulting in shorter recovery times compared to traditional surgery.

Hysterectomy: In some cases, especially if the fibroids are large or causing severe symptoms, a hysterectomy may be recommended. This involves the surgical removal of the uterus and can be done via traditional surgery or minimally invasive techniques.

Potential outcomes and risks vary depending on the procedure chosen and individual factors. Some potential outcomes and considerations include:

Symptom relief: Many women experience significant relief from symptoms such as heavy bleeding and pelvic pain after treatment.

Fertility: Some procedures, such as myomectomy, preserve fertility, while others, like hysterectomy, result in infertility. If fertility preservation is a concern, discuss this with your doctor before choosing a treatment option.

Recurrence: Fibroids may recur after treatment, particularly if the uterus is preserved (e.g., myomectomy or UAE).

Complications: All surgical procedures carry risks, including bleeding, infection, and damage to surrounding organs. Discuss the potential risks and benefits of each procedure with your doctor.
