blockage in bile duct

first of all there was blockage in bile duct, & patient undergone gastroscopic surgery and stain was inserted in left bile duct but it doesn't went in right bile duct. Before surgery the level of billurubin was 6.2 but now after 1.5 month its level 2.2 while the normal level is 1.2 Now doctor had suggested to undergo surgery and remove the blockage in right bile duct by full open surgery. Hence is it necessary to operate or single open bile duct will do the job? |

Dear Mr Niraj Surgery is required in this case. The aim the surgery is to obtain a tension-free, mucosa-mucosa anastomosis between unscarred bile duct and the proximal small intestine. Typically a surgery called Roux-en-Y hepaticojejunostomy is performed. While initially a good response occurs in most of the patient. The majority (75%-93%), recurrent strictures occur in 12%-45%. Additionally, surgical morbidity (18%-51%) and mortality (4%-13%) are high. Laparoscopic surgery is also possible but if the surgeon does not have enough laparoscopic experience you should go for open Roux-en-Y hepaticojejunostomy. |
