Dear Chandan Nondi It is not necessary to operate all ovarian cyst. Still, it's important to be watchful associated with a symptoms or changes in the body and to know which symptoms are serious. For those who have an ovarian cyst, you may experience the following signs and symptoms: * Menstrual irregularities * Pelvic pain - a continuing or intermittent dull ache that could radiate for your back and thighs * Pelvic pain shortly before your period begins or simply before it ends * Pelvic pain while having sex (dyspareunia) * Nausea, vomiting or breast tenderness much like that experienced while pregnant * Fullness or heaviness in your abdomen * Pressure on your rectum or bladder - difficulty emptying your bladder completely The signs and symptoms that signal the requirement for immediate medical assistance include: * Sudden, severe abdominal or pelvic pain * Pain accompanied by fever or vomiting Investigations & Diagnosis To recognize the type of cyst, your physician may perform the following procedures: Pregnancy test. A positive pregnancy test might point to that the cyst is a corpus luteum cyst, which can develop once the ruptured follicle that released your egg reseals and fills with fluid. This will also assistance to rule out a tubal pregnancy if it's negative. Pelvic ultrasound. Within this painless procedure, a wand-like device (transducer) is used to send and receive high-frequency sound waves (ultrasound) through your pelvic area, creating a picture of the uterus and ovaries on a video screen. This picture may then be photographed and analyzed because of your doctor to verify the existence of a cyst, help identify its location and see be it solid, filled with fluid or mixed. A typical USG picture of the multiloculated cysts are shown here. Laparoscopy. Using a laparoscope - a slim, lighted instrument inserted to your abdomen via a small incision - your physician can see your ovaries and take away the ovarian cyst. CA 125 blood test. Blood levels of a protein called cancer antigen 125 (CA 125) often are elevated in females with ovarian cancer. Should you develop cysts on the ovaries that's partially solid and you are at high-risk of ovarian cancer, your physician may test the level of CA 125 in your blood to find out whether your cyst might be cancerous. Elevated CA 125 levels may also indicate noncancerous conditions such as endometriosis, uterine fibroids and pelvic inflammatory disease. Complications The following complications are possible: large size could cause pressure on your bladder, rectum or kidney tubes [ureter] rupture is a serious complication twisting can also be serious because it is painful and leads to stoppage of blood supply to that particular ovary [torsion] infertility Treatment of ovarian cysts Treatment depends upon your age, the kind and size your cyst, and your symptoms. Your physician might point to: Watchful waiting. You can wait and be re-examined in one to 3 months if you are inside your reproductive years, you have no symptoms and an ultrasound shows you possess a simple, fluid-filled cyst. Your doctor will probably suggest that you get follow-up pelvic ultrasounds at periodic intervals to see if your cyst is different in dimensions. Watchful waiting, including regular monitoring with ultrasound, is also a common treatment option recommended for postmenopausal women if your cyst is filled with fluid and less than 2 " in diameter. Oral contraceptives. Your physician may recommend oral contraceptives to reduce the chance of new cysts developing in future menstrual cycles. Oral contraceptives offer the added benefit of significantly reducing your chance of ovarian cancer - the risk decreases the more you take birth control pills. Specific treatments. If you have a cyst because of endometriosis etc the treatment is going to be directed for the primary disease. Surgery. Your doctor may suggest removal of a cyst if it's large, doesn't seem like a practical cyst, is growing or persists through 2 or 3 menstrual cycles. Cysts that cause pain or other symptoms may be removed. Some cysts can be taken off without taking out the ovary in a procedure referred to as a cystectomy. Your doctor may also suggest removal of the main one affected ovary and leaving another intact inside a procedure referred to as oophorectomy. Both procedures may permit you to maintain your fertility if you're still inside your childbearing years. Leaving a minumum of one ovary intact also has the advantage of maintaining a resource of estrogen production. If your cystic mass is cancerous, however, your doctor will advise a hysterectomy to get rid of both ovaries as well as your uterus. After menopause, the chance of a unprecedented cystic ovarian mass being cancerous increases. As a result, doctors additionally recommend surgery when a cystic mass develops around the ovaries after menopause. This surgery may either be carried out in the conventional open manner - laparotomy or like a keyhole non-invasive surgery - laparoscopy. So at this stage we will advice you watchful waiting. Waiting for your valuable advice. Thanks & Best regards Chandan Nondi Dear Chandan Nondi Any cyst can be operated by laparoscopy. There is no contraindication as such and it depends upon the skill of the gynecologist performing the surgery. According to the history of the patient and as you are saying about expensive injection we are expecting Endometrioma (Chocolate Cyst). One of the most frustrating aspects of treating endometrioma with laparoscopic excision is disease recurrence after surgery. Previous medical treatment of endometriosis or large cyst size was a significant factor that was associated with higher recurrence of the disease. Post-operative pregnancy is a favourable prognostic factor. When planning a laparoscopy, gynaecologists should be aware of each individual’s expected likelihood of recurrence as well as her symptoms and desire for current or future fertility. By having information about factors that may be related to a recurrence of ovarian endometrioma, gynaecologists will be able to distinguish patients at risk, optimize the timing of laparoscopy and plan pre- and post-operative management properly. To date, recurrence of ovarian endometrioma after laparoscopy has always been discussed focusing on a single factor, such as the effect of post-operative or pre-operative medication, the method of laparoscopic treatment and the anatomical location. my ultrasound shows 3 cysts measuring 2.6cm 1.6cm and 1.6 cm in left ovary and it is diagonised as ?endometrima kindly advise whether it is dangerous Dear Vijaylakshmi These cysts are small in size and can be just functional cysts. If you are suffering from severe dysmenorrhea than it can be endometriotic cysts. You please write about your age, marital status, no. of children if married and results of any ultrasound or MRI scan. further advice is possible on receiving this information. Dear Madam I am Md. Jakaria, a graduate pharmacist from Chittagong, Bangladesh. Suddenly, my younger sister (14 years old and unmarried) faces pain in her lower abdomen. She has irregular menstruation. Now, a doctor assume that UTI and suggested that laboratory investigation, urine routine exam and ultra sonogram of lower abdomen. In results of microscopic examination of urine Pus cell: Present 2-3/hbf Epithelial cell: Present 1-3/ hbf But other parameters are normal. Results of 1st Ultra sonogram (CSCR diagnostic) Urinary bladder: normal Uterus: Just bulky There is ill-outlined mixed echogenic complex mass, somewhat lobulated in appearance, measuring approximately 5*4.5cm is seen at left adnexa might be of ovarian origin. Then I also done another Ultra sonogram (Chevron diagnostic) Results of second Ultra sonogram (CSCR diagnostic) Urinary bladder: normal Uterus: Normal There is ill-outlined mixed echogenic complex mass, somewhat lobulated in appearance, measuring approximately 4.79*4.22cm is seen at left adnexa might be of ovarian origin. Cul-de-sac: No free fluid collection is seen in Cul-de-sac. Comment: Suggestive of complex left adrenal mass-might be ovarian origin So, therefore, help me further information about this problem and solution. Regards Md. Jakaria Dear Md. Jakaria The complex ovarian mass may be due to endometriosis. You should get some other investigation done including CA 125 and MRI. If this cystic mass is persistent, Irregular, growing and symptomatic, then laparoscopic ovarian cystectomy should be the ideal treatment for her. With regard Nidhi |

Dear Aysha Aapko tension lene ki poi baat nahi hai kyoki aapka ultraound report ekdam sahi hai. Female ko harek majine cyst banta rahta hai jise follicular cyst kahte hain. Filled with fluid, follicular cysts are common in women of reproductive age. Follicular cysts are very rare in prepubescent girls or postmenopausal women. They should not be confused with more serious types of cysts that occur in the ovaries, including cysts from endometriosis or polycystic ovarian syndrome, or tumors. With regard Nidhi |

Dose I have any chance to become a mother....Please suggest me what should I need to do .....Please ....Thanks Dear Tania Your both the ovaries are bigger in size. The normal ovary size is 4.5 cc. It may be due to the ovarian cyst so you need to for the surgery. Laparoscopy surgery can be done for this. A ultrasound can be done which can confirmed you why your ovary size is increased.
