Dear Sulfi Adult chronic immune thrombocytopenic purpura (ITP) can be an autoimmune disorder manifested by thrombocytopenia from the results of antiplatelet autoantibodies and T lymphocyte-mediated platelet cytotoxicity. Multiple studies show that corticosteroid treatment and splenectomy, alone or together, increase platelet counts to safe levels in 60% to 70% of patients. However, there is little change facts about the outcome of ITP patients refractory to splenectomy. To sum up, approximately 75% of patients with RefITP eventually achieved remission, though remission occurred slowly. Patients who did not achieve remission instead experienced a very difficult disease course with significant morbidity and mortality. It will be possible that newer forms of treatment, including rituximab, thrombopoietic factors, and monoclonal antibodies affecting key pathways from the immune system, may alter outcomes because of this extremely resistant subgroup. New therapeutic approaches are incredibly needed. With regards J S Chowhan |
