Dear Sheela As a general rule, most patients who suffer from recurrent reflux symptoms alone or throat discomfort can be managed conservatively with intensive acid suppression and prokinetic agents. Within three month your discomfort will be O.K. A reoperation should only be considered in those with low surgical risk. In contrast, patients who suffer from persistent postoperative dysphagia with or without associated reflux symptoms can usually not be managed conservatively and usually require revisional surgery. In our experience, you should keep patient as medical management is possible in about half of the patients who complain of recurrent or persistent reflux symptoms alone, while remedial surgery is necessary in about 90% of patients with postoperative dysphagia or a combination of dysphagia and reflux symptoms. Please consult your surgeon. The indication for a revisional procedure should therefore be guided by the dominating symptom and the underlying cause of failure. With regard J S Chowhan |
