Detail of treatment is as under: 1. laparoscopic cholecystectomy on 6/2013 using .Everything was normal at the time of operation. 2. After some time around august2013 there a pus discharge near the naval area where laproscopic instrument was inserted. Doctor said that nothing to worry its a infection he prescribed Claribid 500 for the two weeks. After two weeks pus discharged reduced considerably .Doctor said that you should spirit wash the infected area.. 3.In the month of 2/2/14 a sonography of port site performed that show a tract extending upto under surface of skin.Doctor again prescribed Claribid no improvement was there and also two-three nodule are formed around the port site. 4.In the month of August 2014 another sonography was done that show a long tract along with some side tract. We went to another Dr. Sandeep Bhat he adviced Biopsy of said area which performed on 24/8/14.AFB culture of Pus was done that come positive for MYCOBACTERIUM OTHER THAN M. TUBERCULOSIS (SLOW GROWER ) 5. After that doctor prescribed Claribid 500, bd Ciplox500 bd for treatment upto 15.11.2014. after 15/11 Amicacin 500mg. injection (twice a day)was also given from 15/11 to 25/11 besides the antibiotics already taken.Another sonography was done shows regression in tracts was seen. 6.After 25/11/14 doctor prescribed Amicacin injection along with Doxy1 bd upto 17/12/14. 7. From 18/12/2014 only Doxy1 has been taken regularly till 7/3/2015.Another sonography was done on 7.3.15 which shows improvement but tract is still there. 8. From 8/3/2015 doctor prescribed Claribid 500 along with Doxy 1.upto 5/4/15. 9. From 6/4/2015 doctor prescribed SeptronDS in addition to Claribid and Doxy. 10. Today another Sonography was done that shows three blind tracts are still there short of rectum in the umblical region at the site of trocar insertion. Now Doctor advice that we have to remove the tracts Surgically. Please advice it is right decision or not. Ragards Regars |

Dear Mrs. Raman Bhat You have not mentioned about the biopsy report. You may get another biopsy from the edge of the sinus and if the biopsy report shows any granulomas or the giant cell reaction, then there is very strong possibility of port site tubercular infection . In this situation anti tubercular treatment is to be started. So before going for excision biopsy of the sinuses the biopsy from the edge is advisable. Thanks With Regards Dr j S Chowhan |

Sir, My Biopsy was done in the month of August 2014 that came positive for MYCOBACTERIUM OTHER THAN M. TUBERCULOSIS (SLOW GROWER).After that ATT treatment was started and is still on.No discharge is taking place from the sinus they all are healed up and they all are closed. Now Doctor advice that we have to remove the tracts Surgically. Please Advice Sir. Regards Raman Dear Raman If the tracts have dried up this means that tubercular infection is cured. Sinogram may be done to see whether the tract is still patent. And if patent then excision of tract may be planned. Thanks With Regards Dr J S Chowhan |
