I had a surgery last year, July 2019
Endometriosis stage IV
Endometrioma ( bilateral) cystectomy
Hysteroscopic Myomectomy ( two fibroids on top of each other)
And appendectomy
Before surgery, I had no problems
But my cysts were large and listened to Dr to remove them
My period was always 28 days - normal but a little heavy 5 days

After surgery. I am bloating, have inflammation, feel heavy and pressure on the uterus
My right ovary has pain sometimes

My period is very light now . 1 day only spotting before and after
I feel I am in period for a few days but there is no blood
I think damaged my uterus?
Or maybe my Ovaries are damaged? I don't know

Is there anyway to improve period blood flow? Longer and blood comes out easier


Also want to know if there is high possibility of Asherman's syndrome and how i can prevent it

The recurrence rate of ovarian endometrioma after conservative laparoscopic surgery has been reported to be 29–56% at 2 years and 43% at 5 years. When postoperative medical treatment was introduced, the endometrioma recurrence rate substantially declined to 3–11% at 2 years and 6% at 5 years. As a complication there may be partial ovarian damage. Cause of your pain may be adhesion. I will suggest you get one MRI pelvis and one recent ultrasonography done. Please let us know also that, Did you completed hormonal therapy after surgery of endometrioma.

My concern is to Find out the reason for my light period after surgery. As the surgery report ( date: July 9, 2019).

I had :

Laparoscopic excision of severe endometriosis, ovarian cystectomy ( bilateral endometriomas), lysis of adhesions, appendectomy and Hysteroscopic Myomectomy

I am suspected that Myomectomy created some problems in the Uterus ( like Ashermans syndrome). I understand and Know that the Cystectomy of endometrioma could have damaged the ovary's function and Fewer hormones.

I do not know the reason for the change in my cycle and my surgeon does not relate anything but he says that my age. Howcome within a month After surgery all these symptoms showed

Please Note: I had no symptoms before surgery ( less than a year ago). The reason Dr convinced me to do the surgery. Is that one of the ultrasound made some inflammation in my tube, but it was wrong
I had no pain nor complain, I was fine, I did not know I have 10 cm endometrioma. That showed in my annual checkup. Anyhow, the past is past, and I can not undo. Please help me to know how I can have a normal period ( my normal used to be 5 days -and heavy). Please help me to find out how I can have good care of uterus and ovaries i know both damaged.

10 cm endometrioma itself is an indication of surgery. In many situations less bleeding during periods are not an issue. To know the ovarian size and to rule out intrauterine adhesion ultrasonography will not help. That's why we suggested you to get one MRI done. You can also get a few hormone levels checked to know how good ovaries are functioning. For this you should get LH FSH and Estradiol level tested. You also need a thorough pelvic examination so we will advise you to consult a local gynecologist as online advice has a lot of limitations.

Yes I am in a waiting list to do MRI and the test is done, it was on my first day of the period ( not third day) are attached. My question is about Fibroid as i had Hysteroscopic Myomectomy. And why my period flow changed after surgery
Is it because of the damage in my ovaries, blood vessels, or because of myomectomy and I lost some part of my uterine lining thickness?
What is your opinion?
I just want to know the reason of my light period that started after surgery.

We have seen your hormonal report and it looks good. There is no any abnormality. Let us wait for the MRI report. The question of light period is unexplained right now. After hysteroscopy endometrial lining are destroyed up to some extent but in most cases it regenerates. If there is any problem with Asherman syndrome or destruction of lining it will come in MRI.

Please find attached two ultrasound results showing endometrium is visualized along whole uterus. 1st exam was done in June 2019 1 month before surgery. 2nd exam was done in Sept 2 months after surgery
Does it look like the endometrium is a problem ? or some abnormality there? now is almost 11 months after surgery. do you think I can fix the problem if the whole cavity is distorted?

Ultrasound showing endometrium is visualized along with the whole uterus. Little irregular endometrium is not a big issue. However it looks like your endometrioma is again growing and it is destroying your tubes. Subserous fibroid and adenomyosis is also an issue. The recurrence rate of ovarian endometrioma after conservative laparoscopic surgery has been reported to be 29–56% at 2 years and 43% at 5 years. When postoperative medical treatment was introduced, the endometrioma recurrence rate substantially declined to 3–11% at 2 years and 6% at 5 years. In our opinion you should consult a good gynecologist and start medical treatment.

It is 11 months past my surgery. I am afraid Haemorrhage can occur in submucous fibroids Myomectomy.
I had no problem having two small fibroids. But since then I have pressure in my uterus, bloating, kind of water retention, and very light period.
